Abstract
Sudden cardiac death is one of the major causes of mortality in western countries, with an incidence of 500 000 sudden deaths per year in the United States and 400 000 sudden cardiac deaths per year in Europe [1, 2]. It has been shown that the risk of sudden cardiac death increases with the severity of left ventricular (LV) dysfunction [3-5]. Although treatment for congestive heart failure (CHF) has improved markedly during the last 20 years, the prognosis is still poor in patients with severe heart failure, with an annual mortality of 50% [6]. Different studies indicate that the implantable cardioverter-defibrillator (ICD) prevents sudden cardiac death in patients with life-threatening ventricular tachyarrhythmias and impaired LV [7-9]. However, the extent of the clinical benefits of long-term survival is unclear, and it is unknown whether patients with different degrees of heart failure will benefit from ICD implantation or not.
