Riassunto

L’interazione tra attivazione autonomica e rischio cardiovascolare è nota e sostenuta da un’ampia letteratura scientifica. In particolare, l’attivazione simpatica gioca un ruolo determinante nel facilitare l’insorgenza di aritmie maligne nel miocardio ischemico [1]. La conseguenza e riprova di questa fondamentale nozione di fisiopatologia è il fatto che gli unici interventi efficaci nel ridurre il rischio di morte aritmica dopo infarto miocardico e in pazienti con disfunzione ventricolare sinistra sono i farmaci che bloccano i recettori beta-adrenergici e i farmaci inibitori l’enzima di conversione dell’angiotensina (ACE inibitori) [2]. In altre parole, l’intervento farmacologico che modula la risposta neuroumorale all’ischemia miocardica è l’unico in grado di migliorare la prognosi di pazienti destinati altrimenti a morire a causa di eventi aritmici. Controparte di tale nozione è il forte potere predittivo positivo degli indici autonomici derivati dall’analisi del controllo riflesso della frequenza cardiaca, vale a dire la sensibilità barocettiva e la variabilità della frequenza cardiaca
