Riassunto

La ventilazione meccanica costituisce il principale strumento di supporto vitale che l’intensivista pediatrico può utilizzare per far fronte alle gravi patologie respiratorie del bambino. È importante tenere presente che la ventilazione è un presidio terapeutico di supporto, in attesa che i trattamenti specifici per la patologia in atto manifestino il loro effetto. La scelta della tecnica e della modalità ventilatoria può tuttavia influenzare in modo significativo il decorso della malattia polmonare stessa, oltre che l’outcome del paziente. Il medico deve quindi compiere scelte che consentano un compromesso tra adeguati scambi gassosi e strategie di protezione del polmone. Gli studi clinici randomizzati e controllati, che possono fornire all’intensivista pediatrico indicazioni e protocolli specifici basati sull’evidence based medicine, sono pochi e obbligano ad adattare al bambino informazioni derivate da studi sull’adulto.
