L’errore medico e il danno al paziente sono stati descritti e studiati per oltre un secolo. A eccezione di pochi isolati pionieri, tuttavia, le professioni mediche e infermieristiche non sono parse consapevoli dell’entità e della gravità del problema o, se anche lo erano, non si sono dimostrate preparate a riconoscerlo. Uno dei grandi successi degli ultimi dieci anni è che l’errore medico e il danno al paziente sono oggi riconosciuti e discussi pubblicamente dai professionisti della sanità, dai responsabili politici e dal pubblico.
