Nel 1860 Von Gubler descrisse per la prima volta un’oftalmoplegia dolorosa presentando il caso di un uomo di 35 anni che soffriva di attacchi dolorosi ricorrenti, di tipo emicranico, associati ad oftalmoplegia transitoria di durata variabile, da una settimana ad un mese. Il paziente morì dopo un episodio doloroso e l’esame autoptico rivelò la presenza di un processo meningitico della base cranica. Sono stati descritti in seguito molti casi analoghi: Mauthner ne riportò 14 nel 1889, Ballet 22 nel 1896, Mingazzini 60 nel 1897.
