Ventilazione in corso di trauma cranio-cerebrale ed elevata pressione intracerebrale

I traumi cranio-cerebrali devono essere distinti in primari e secondari. I primi sono causati da forze di taglio, da trazione o da impatto. Conducono a contusioni cerebrali focali e lesioni reversibili o irreversibili. I secondi, sono da ricondursi ad ipossia-ischemia cerebrale causate da contusione ed emorragia focale, ma anche diffusa, per aumento posttraumatico della pressione intracerebrale. Altre cause importanti di lesione cerebrale secondaria sono:

  • Ematoma intracranico;

  • Alterazioni metaboliche;

  • Ipossia, ipercapnia ed ipotensione;

  • Iperglicemia;

  • Ipertermia;

  • Alterazioni della barriera ematoencefalica
