La sindrome da distress respiratorio acuto (ARDS) è una condizione clinica devastante, relativamente comune, che puÒ colpire sia pazienti “medici” che “chirurgici” [1, 2]. La definizione ed i criteri diagnostici attualmente accettati (Tabella 1), proposti nel 1994 nel corso di una consensus conference americano-europea, hanno introdotto significativi vantaggi tra cui la semplificazione della diagnosi clinica, ed il fatto che venga riconosciuta e specificata l’esistenza di due stadi di gravità della sindrome sulla base dello stato degli scambi gassosi [3]. Nella forma più “lieve”, definita acute lung injury (ALI), la ipossiemia tipica della sindrome è meno severa (PaO2/FiO2 tra 300 e 200) che nella vera e propria ARDS (PaO2/FiO2< 200) [3].
