La diagnostica del versamento pleurico è stata una delle prime indicazioni alia videotoracoscopia. La semplicità di esecuzione e l’elevato potere diagnostico della metodica nell’applicazione specifica, rendono tuttora questa indicazione la più universalmente diffusa. Ciò anche in ragione del fatto che il versamento pleurico rappresenta una patologia comune, che spesso rimane di diagnosi incerta, nonostante le toracentesi ripetute e la biopsia pleurica a cielo chiuso: circa il 20% dei versamenti pleurici risulta infatti non diagnosticato [1]. La videotoracoscopia permette inoltre di associare al tempo diagnostico ulteriori manovre operative, come lo sbrigliamento del cavo pleurico nell’eventualità di versamento saccato, la lisi delle aderenze pleuropolmonari, il trattamento del versamento mediante le tecniche per ottenere la pleurodesi, una pericardiotomia in caso di concomitante versamento pericardico, biopsie polmonari o mediastiniche se presenti lesioni associate.
