La resezione polmonare atipica od economica è l’asportazione di una porzione di parenchima polmonare effettuata senza seguire i normali piani anatomici ma modellata sulla forma e sulla estensione della lesione da resecare. Fino agli anni ’90, l’intervento veniva effettuato esclusivamente in toracotomia postero-laterale, antero-laterale od ascellare. Lo sviluppo delle tecniche videotoracoscopiche, ha rivoluzionato la pratica chirurgica quotidiana in chirurgia toracica. Dalla classica toracoscopia di H.C. Jacobaeus, con finalità per lo più diagnostiche, si è arrivati ad una toracoscopia esclusivamente chirurgica, largamente impiegata per il trattamento di molte patologie toraco-polmonari. Il perfezionamento della tecnica, parallelamente allo sviluppo della strumentazione, sempre più sofisticata, ha reso possibile la diffusione della metodica e l’ampliamento delle indicazioni in chirurgia toracica, con applicazione della VATS (Video-Assisted Thoracic Surgery) nel 25–35% dell’attività di chirurgia toracica.
