L’empiema pleurico consiste nella raccolta di materiale purulento all’interno della cavità pleurica. Ippocrate (Fig.1) fu il primo a descriverne i sintomi ed i segni quali febbre settica, dolore toracico, tosse produttiva ed il decorso spesso infausto, così come fu il primo a tentare un trattamento mediante una rudimentale “open thoracostomy” [1]. Tale procedura rimase la sola ed unica opzione terapeutica in questa patologia fino al XIX° secolo quando venne descritta, ma non adottata immediatamente, la procedura del drenaggio toracico chiuso. Questa tecnica divenne ampiamente utilizzata durante l’epidemia d’influenza del 1917–1919 quando il drenaggio toracico aperto era funestato da una mortalità superiore al 70% [2]. Tale mortalità era probabilmente dovuta all’insufficienza respiratoria prodotta dall’esteso pneumotorace che si creava con il drenaggio toracico aperto, in particolare nelle infezioni da Streptococcus Haemolyticus che producendo streptokinasi riduce la formazione di aderenze pleuriche.
