Il concetto di bronchite cronica negli ultimi anni si è notevolmente modificato. Nel 1961 l’Organizzazione Mondiale della Sanità ha definito la bronchite cronica come una “condizione morbosa caratterizzata da un persistente aumento delle secrezioni bronchiali, con tosse produttiva quasi quotidiana per almeno tre mesi l’anno per due anni consecutivi” quando altre cause di tosse produttiva sono state escluse. Tali requisiti escludono anche cause transitorie di tosse e aumento delle secrezioni bronchiali, quali la bronchite acuta. Nel progetto GOLD (Global Initiative for Chronic. Obstructive Lung Disease) del 2003 la bronchite cronica è considerata come uno stadio a rischio di BPCO o stadio 0, che presuppone un monitoraggio clinico-strumentale stretto ed interventi sui fattori di rischio per la possibile progressione in BPCO.
