La malattia oncologica e gli accidenti cerebrovascolari sono rispettivamente la seconda e la terza causa di morte nei paesi occidentali, la loro concomitanza nel medesimo individuo non è pertanto sorprendente. In assenza di studi prospettici che quantifichino il rischio di stroke nei pazienti con cancro, dobbiamo considerare che le lesioni cerebrovascolari sono il secondo reperto anatomopatologico in ordine di frequenza (dopo le metastasi) negli individui che muoiono di cancro. Nello studio autoptico retrospettivo di Graus et al [1] su 3426 pazienti con cancro avanzato uno stroke era presente nel 14,6% dei casi ed in 255 (7,4%) pazienti questo evento era stato sintomatico. Le lesioni ischemiche ed emorragiche erano egualmente ripartite, anche se queste ultime erano più frequentemente sintomatiche. Gli accertamenti diagnostici nei pazienti con stroke criptogenetico possono non raramente portare al primo riscontro di neoplasia occulta [2].
