Evaluation of the Graft and Tissue Preparation for Modern Endothelial Keratoplasty

During the past decade, endothelial keratoplasty (EK) has replaced penetrating keratoplasty for the treatment of corneal endothelial dysfunction. Initial iterations of EK were performed in the 1960s by Drs. Barraquer [1] and Polack [2], who created a hinged anterior corneal flap that could be retracted for trephination, followed by transplantation of an endothelial or stromal graft, and ultimately with suturing of the corneal flap. Although innovative, this technique was not widely used because it suffered from many of the same corneal surface issues of penetrating keratoplasty, including the need for surface corneal suturing.
