The focus of this chapter is on the management of locally advanced disease, lymphadenectomy and treatment of recurrent disease in patients with well-differentiated thyroid cancers.

Commentary
This chapter on the management of advanced thyroid cancer is concise and well written. It contains all the information required for daily practice.
The management of advanced thyroid cancer represents a surgical challenge because of the complex anatomy of the neck region and the gland’s close topographic relationship to adjacent organs. The optimal goal is to improve overall survival (or at least progression-free survival). This can be achieved by an operation that encompasses radical removal of the entire tumour and invaded structures. Accurate preoperative staging to determine the resectability and assess the extent of resection is a conditio sine qua non in surgical planning. The approach presented in this chapter corresponds to the way our group treats patients with advanced thyroid cancer.
