The Polyposis Center of Japan had registered 970 cases and 631 families of familial adenomatous polyposis (FAP) up to Oct. 1989. The fusion of families which were registered independently from different institutes was recognized in 104 (16.5%) families. Our survey revealed 1738 affected persons and 4475 persons at risk as carrier of the FAP gene. The frequency of cancer was less in the call-up cases (21%) than the proband cases (60%). Periampullary carcinoma and desmoid tumor were the two most serious extra colonic malignancies. We need to pay attention to the bile duct and to development of desmoid tumor after colo-rectal operation.
