Intraportal Endovascular Ultrasonography for Pancreatobiliary Carcinoma

Abstract

Intraportal endovascular ultrasonography (IPEUS) was performed to diagnose the portal vein invasion of pancreatobiliary carcinoma. The study group consisted of 15 patients, six men and nine women, with a mean age of 65.5 years who underwent IPEUS between Feb 1992 to Dec 1992. Basically, the wall of portal vein was visualized as echogenic band with a thickness 0.5mm to 1.0mm. When this echogenic band was seen, portal invasion was diagnosed as negative. When this band was destroyed, portal invasion was diagnosed as positive. In 14 out of 15 patients, portal invasion of the tumor was diagnosed correctly. In one case of bile duct cancer of hepatic hilum, tumor invasion of right hepatic artery could also be diagnosed. This finding could not be detected by conventional imaging technology such as CT, angiography. IPEUS was considered to give important information about pancreatobiliary carcinoma.
