Left ventricular assist device: Experimental and clinical study

Summary

Recently, intra-aortic balloon pumping (IABP) has been widely employed clinically as a means of mechanical circulatory assistance. However, there have been a few patients who could not be saved from severe heart failure after a cardiac operation in spite of IABP assistance. An evaluation of the left ventricular assist device (LVAD) was experimentally carried out for profound heart failure after acute myocardial infarction, which was produced by multiple ligations of the coronary arteries. Consequently, the efficacy of the LVAD was clearly confirmed hemodynamically in comparison with another means of mechanical assistance such as VA bypass and IABP + VA bypass.

On the basis of our experimental studies, the LVAD was successfully employed in a 68-year-old female patient who underwent closure of a ventricular septal rupture (VSR) and AC bypass for VSR following acute myocardial infarction (AMI). She was weaned from the LVAD and IABP 7 days after surgery. Our experience concerning the LVAD in mechanical circulatory assistance is reported in detail.
