Summary

The Japanese Clinical Practice Guideline for Benign prostate Hyperplasia was established in March 1999. The 1999 Guideline was developed according to the methodology of evidence-based medicine by a panel consisting of 10 academic and 10 practicing urologists and 2 epidemiologists, in cooperation with the Japanese Urological Association. The 1999 Guideline consisted of diagnostic and treatment sections. Diagnosis is made according to the International Prostate Symptom Score (I-PSS), the quality-of-life (QOL) score, the maximal flow rate on uroflowmetry, the volume of residual urine, and the estimated prostate volume by ultrasonography. According to the severity of disease, patients are classified into three categories: mild, moderate, and severe. The treatment modalities are watchful waiting, medication, minimal invasive therapy, and surgery. “Mild” patients are recommended to select either watchful waiting or medication; “moderate” patients choose medication, minimal invasive therapy, or surgery; and “severe” patients are recommended to undergo minimal invasive therapy on surgery. The background for the development of the 1999 Guideline is also described.
