Abstract

The majority of colorectal cancers arise from adenomas, following the “adenoma-carcinoma sequence.” Accordingly, resection of all neoplastic polyps (“clean colon”) can be expected to reduce the cumulative incidence of colorectal cancer by 76–90 %. However, attempts to achieve a “clean colon” may result in an increase in unnecessary polypectomies, making differentiation between neoplastic and nonneoplastic polyps extremely important.
