Randomisierte Studien zur Art der Operation bei diffuser Peritonitis fehlen. Kontroverse Diskussionen, insbesondere zur Abgrenzung von programmierter Relaparotomie gegenüber einer Revision on-demand entbehren daher einer solide Grundlage an Literaturdaten. Vorteilhaft erscheint dem gegenüber die Flexibilisierung der chirurgischen Therapie befundadaptiert bei demselben Patienten während des oft langwierigen Krankheitsverlaufs. Wesentlich ist die primäre Herdkontrolle, da diese in den allermeisten Fällen die definitive Therapie darstellt. Ist die Herdkontrolle bei der Erstoperation unmöglich, muss eine programmierte Relaparotomie durchgeführt werden.
