Abstract

Femoro-popliteal reconstructions are standard procedures in vascular surgery, being performed in clinical stages Fontaine III and IV. Apart from vein grafts, as alloplastic bypass materials mainly ePTFE and Dacron are available. Five-year patency rates of 70–80% are documented for vein bypasses in the femoro-popliteal region; with alloplastic material, patency rates of 60% are reached. Procedures like half-closed ring desobliterations have fulfilled their expectancy. Alternative graft materials (umbilical vein, xenograft) are used sporadically. Even after 50 years of bypass surgery of the femoral region the quest for the ideal bypass material has not ended. Additional conservative as well as pharmacological therapies still contribute to successful treatment of peripheral arterial occlusive disease and are indispensable in secondary prophylaxis. Treatment of the vascular patient ought to occur in interdisciplinary vascular centers.
