In zahlreichen Studien wurde die Beziehung zwischen körperlicher Aktivität oder körperlicher Fitness und kardiovaskulärer Mortalität und Morbidität untersucht. Die Ergebnisse der diesbezüglichen epidemiologischen Forschung resultieren insbesondere in der Primärprävention aus Beobachtungsstudien im Sinne von Kohortenstudien, die als prospektive Längsschnittuntersuchungen durchgeführt worden sind. Aus logistischen und methodischen Gründen sind randomisierte kontrollierte Aktivitätsstudien über mehrere bis viele Jahre nicht machbar.
