Zum Zeitpunkt der Erstimplantation liegt das Durchschnittsalter von Schrittmacherpatienten bei 75 Jahren [3]. Patienten, die einen implantierbaren Kardioverter/Defibrillator erhalten, sind mit 61 Jahren deutlich jünger [5] und wahrscheinlich körperlich aktiver als Schrittmacherpatienten. Eine landesweite Umfrage unter US-amerikanischen Elektrophysiologen [12] belegte, dass nur eine Minderheit von 24% der in der ICD-Nachsorge tätigen Ärzte ausschließlich Patienten betreut, deren körperliches Aktivitätsniveau auf S1/D1 (z. B. Golfspielen, Bowling, s. auch Tabelle 4.2, S. 80) beschränkt bleibt. Obwohl laut Konsensusempfehlungen der Fachgesellschaften [7, 17, 22] das Vorhandensein eines ICD unabhängig von der Indikationsstellung und der kardialen Grunderkrankung für die Teilnahme an sportlichen Wettkämpfen, die über ein geringes Belastungsniveau (> S1/> D1) hinausgehen, disqualifizieren sollte, sieht die Praxis anders aus: Knapp die Hälfte aller befragten Ärzte betreuen auch Patienten, die sportliche Aktivitäten von mindestens moderatem Belastungsniveau ausüben und 42% aller nachsorgenden Kardiologen sind mit wenigstens einem ICD-Patienten konfrontiert, der Wettkampfsport betreibt. Auch hier überwiegen Sportarten mit moderatem und hohem Belastungsniveau wie Basketball, Laufen, Schwimmen und andere Ballsportarten. Entsprechend dem geringen Anteil von 18–28% aller Deutschen, die im Alter zwischen 70 und 79 Jahren noch irgendeine regelmäßige sportliche Aktivität ausüben [19], kann davon ausgegangen werden, dass das ältere Klientel der Schrittmacherpatienten sich grundsätzlich seltener und weniger intensiv belastet.
