Am Rückfuß gibt es eine Vielzahl von Gelenks-, Weichteil- und Knochenpathologien. Sofern kein intraartikulärer Schaden vorliegt, haben posteromediale Sprunggelenksbeschwerden am häufigsten eine Störung der Tibialis posterior-Sehne zur Ursache [21]. Wird durch eine konservative Therapie keine Besserung erzielt, können Erkrankungen der Tibialis posterior-Sehne operativ behandelt werden [19]. Eine posteromediale Überbeanspruchung und posttraumatische Beschwerden sind bei Balletttänzern und Fußballspielern am häufigsten durch eine Tendovaginitis der Flexor hallucis longus-Sehne und/oder einem posterioren Impingementsyndrom bedingt. Posterolaterale Sprunggelenksbeschwerden sind am häufigsten durch Erkrankungen der Peronealsehnen verursacht, wie z.B. Tendovaginitis, Teilruptur und Sehnen-Subluxation.
