Zusammenfassung und Aussicht
Die Vorteile der PDT liegen in der nicht invasiven und vergleichsweise einfachen Durchführung, der Wiederholbarkeit der Behandlung und den ausgezeichneten kosmetischen Resultaten. Die Nachteile umfassen die geringe Tiefenwirkung der topischen PDT, welche diese Therapieform auf oberflächliche Hauttumoren und entzündliche Dermatosen mit vorwiegend epidermaler Mitbeteiligung beschränkt. Die gerade bei großflächiger topischer PDT bei der Hälfte der Patienten während der Belichtungsphase auftretenden brennenden Schmerzen bedürfen einer ausreichenden Analgesie. Schwerwiegende Nebenwirkungen oder Komplikationen sind bislang nicht beobachtet worden.
Durch die Erprobung neuer Photosensibilisatoren und die Entwicklung neuer Lichtquellen mit Leuchtdiodentechnologie oder gefiltertem Licht wird die PDT in Zukunft voraussichtlich eine breitere Anwendung finden. Zudem wäre die Entwicklung systemisch zu verabreichender, jedoch selektiver Photosensibilisatoren mit kurzer Halbwertzeit wünschenswert. Für die topische PDT könnten Photosensibilisatoren mit Absorptionsmaxima im nahen Infrarotbereich die Behandlung von dickeren (τ3 mm) Tumoren ermöglichen. Im Hinblick auf eine Evidenz-basierte Medizin bedarf es jedoch in erster Linie größerer kontrollierter Studien, um den Stellenwert der PDT in der Dermatologie darzulegen.
