Schizophrene Patienten im Landeskrankenhaus (Stationäre Inanspruchnahme einer regionalen Psychiatrie)

Untersuchung der administrativen Inzidenz 1984–1988

Zusammenfassung

Seit 1986 wird im LNKH Valduna ein Erfassungsbogen für patienten- und behandlungsrelevante sowie sozialpsychiatrische Daten verwendet. Bei Abschluß der individuellen Krankengeschichte werden Entlassungsbefunde dokumentiert: neben den Diagnosen werden insgesamt 30 andere Variable erfaßt. In dieser Arbeit werden die Diagnosen, nach ICD 9 295.x, Schizophrenien, in verschiedenen Variablen verglichen. Es zeigen sich dabei zum Teil große Unterschiede in der Geschlechtsverteilung, im Familienstand, in der Aufenthaltsdauer und bei den Berufen. Bei den Schizophrenien zeigt sich die höchste Ledigenrate und Rate von Hilfsarbeitern. Berentungen treten bei allen endogenen Psychosen besonders häufig in Erscheinung, wobei deren Zustandekommen jeweils kritisch zu hinterfragen wäre. Die Bedeutung von (auto-)aggressivem Verhalten, familiärer Komponenten und möglichen life-events wird diskutiert.

Summary

The schizophrenic patients and the psychiatric hospital (administrative incidences 1984–1988). This study reflects regional psychiatric epidemiological data, as recorded by admissions/dismissals from the regional psychiatric hospital, the Landesnervenkrankenhaus Valduna. With 420 beds it is the only psychiatric unit for the Austrian province of Vorarlberg, a catchment-area of approximately 330,000 inhabitants. The data concern hospitalized patients only and were collected at discharge on standardized forms. This paper covers the period 1984–88 and the diagnoses ICD-9 295.x. We report on interrelationships of 1261 patients main diagnoses and demographic, social, educational, vocational an other data. Contrary to the catchment area’s sex-distribution, more male than female patients were admitted (55.3% m, 44.7% f), overall 48% were first-admissions. Diagnosespecific sex distribution patterns are apparent: In schizophrenia the ratio is nearly equal, affective diagnoses show a preponderance of female patients. As the hospital is concerned with adults only, the lowest age bracket ranges from 21–30 years, predominant in the endogenous psychoses. The highest rate of singles (708 = 69%) was registered in schizophrenics, for divorced persons in the alcoholics’ group. General practitioners are the most important source ot referrals and also focus for discharged patients, although the area has a per capita density of specialists of 1/20,000. Austria itself has one of the world’s best developed social-security systems and the province is reknown for its dense social network. General hospitals as main referrers of suicide-attempters (of various causes) indicate a deficit of some basic services. This is also stressed by the high number of unemployed and invalid-pensioners among psychotic patients. Rehabilitation and social measures, social care and divers forms of living-arrangements are needed in various degrees for this group of handicapped. Low-grade workers make up 54% of our patients and frequently are schizophrenics. As expected housewifes predominate the affective psychoses (44% of the group), in which we also registered a high rate of (invalid) pensioners.
