Zusammenfassung
Bei vielen Patienten mit Asthma bronchiale ist ein gastroösophagealer Reflux (GER) zu finden. Die Beobachtung, daß durch Behandlung des GER auch die asthmatische Ventilationsstörung beeinflußt wird, läßt einen kausalen Zusammenhang zwischen den beiden Erkrankungen vermuten. Der durch Säureinstillation in das untere Ösophagusdrittel bei einem Teil der Asthmatiker auslösbare Bronchospasmus ist jedoch zu gering, um klinisch von Bedeutung zu sein. Daher wird als Mechanismus für den Zusammenhang zwischen niedrigem pH im Ösophagus und Tonus der Bronchialmuskulatur eine vagal mediierte Steigerung der bronchialen Hyperreagibilität angenommen. Dadurch könnte ein GER den Patienten gegenüber anderen bronchokonstriktorisch wirkenden Stimuli empfindlicher machen. Da Asthma per se, aber auch die Mehrzahl der für die Asthmatherapie eingesetzten Medikamente, GER begünstigt, kann bei diesen Patienten ein Circulus vitiosus zwischen Asthma und GER entstehen.
