Zusammenfassung
Im Zentrum der gestellten Thematik stehen zwei unscharf definierte Syndrome: „Sepsis“ und „disseminierte intravasale Gerinnung“ (DIG). Die Variationsbreite der Sepsis nach klassischer Definition — mit positiver Blutkultur — reicht von der fulminanten Meningococcensepsis bis zur Endocarditis lenta. In der Mitte steht die häufige abdominelle Sepsis bei Abszeßbildung oder Peritonitis. Nach heutigem Verständnis spielt die mit geeigneter Methodik nachweisbare Endotoxinämie mit Schocksymptomatik, aber negativer Blutkultur, als „septischer Schock“ eine zentrale Rolle. Diesen Zustand gibt es gleichfalls bei grampositiver Infektion, Virämie, Pilzsepsis und — klinisch betrachtet — bei der Malaria tropica. Verschiedenartig sind auch die Bezeichnungen einer DIG: Hier genannt seien lediglich Hypofibrinogenämie oder Defibrinierungssyndrom, Mikroemboliesyndrom und Verbrauchskoagulopathie. Die nachfolgenden Ausführungen sind daher größtenteils als allgemeindeskriptiv zu verstehen. Gemeinsamkeiten ergeben sich am ehesten durch Betrachtung der pathologischen Endothelfunktion. Als Summeneffekt viefältiger Syntheseleistungen sind normale Endothelzellen antikoagulatorisch wirksam.
