Zusammenfassung
Die wesentliche Indikation für eine Bluttransfusion ist neben der Wiederherstellung des Blutvolumens die Restituierung der Sauerstofftransportkapazität. Daher werden in aller Regel nach dem Prinzip der Hämotherapie nach Maß vornehmlich bei der Komponententherapie Erythrozyten in Form von Erythrozytenkonzentraten übertragen. Durch die Entfernung eines Teils des PlasmaStabilisator-Gemisches sowie nichterythrozytärer Zellen wird somit in vielen Fällen das Risiko der Übertragung überflüssiger Ballaststoffe und teils auch schädlicher Blutbestandteile verringert (Abb. 1). Dennoch enthalten Erythrozytenpräparate der Regelversorgung je nach Lagerungsalter intakte oder fragmentierte Granulozyten, Lymphozyten und Thrombozyten, die als Träger von Antigenen von besonderer Bedeutung sind (Abb. 2). Zwar verlieren gerade diese korpuskulären Elemente mit Ausnahme der Lymphozyten sehr rasch ihre Funktion und sind somit in gelagerten Blutkonserven nur noch als Ballast zu betrachten, aufgrund ihrer intakten antigenen Eigenschaften können sie jedoch zur Sensibilisierung der Empfänger gegen Antigene des HLA-Systems oder gegen leukozyten- sowie thrombozytenspezifische Determinanten führen [4, 7].
