Zusammenfassung
Das aus Podophyllinextrakten (Podophyllum peltatum bzw. Podophyllum emodi) isolierte Podophyllotoxin ist seit mehreren Jahrzehnten als aktiver Mitoseblocker bekannt (1, 2). Aufgrund seiner ausgeprägten Toxizität mußten für eine klinische Verwendung weniger toxische Derivate entwickelt werden. Bei den semisynthetischen Glukosid- verbindungen VP 16-213 (Etoposid) und VM 26 (Teniposid) blieb einerseits die zytotoxische Wirkung erhalten, andererseits zeigten sie eine bessere Verträglichkeit, sodaß von ihnen klinisch ein hoher therapeutischer Index erwartet werden konnte (3, 4, 5). Der molekulare Wirkungsmechanismus von VP 16-213, der für den zytotoxischen Effekt verantwortlich ist, konnte bis heute nicht endgültig aufgeklärt werden. Eine Hemmung des Nukleosidtransports in die Zelle sowie eine Hemmung der DNS- und RNS-Synthese wurden beobachtet. Die Synthesehemmung von spezifischen Proteinfaktoren der G2-Phase dürfte mit der prämitotischen Blockierung von Zellen im Zellzyklus im Zusammenhang stehen. Die zytozide Wirkung von Etoposid ist auf Zellen der S- und G2-Phase am größten (6, 8). An einer Reihe von Tiertumoren konnte VP 16-213 als wirksames Zytostatikum ausgewiesen werden, wobei eine mehrmalige Applikation der einmaligen Verabreichung überlegen war (3). Unter Berücksichtigung der Ergebnisse präklinischer und klinischer Toxizitätsstudien hat sich die intravenöse Applikation von VP 16-213 in einer Dosis von 60 bis 120 mg/m2 an 3 bis 5 aufeinanderfolgenden Tagen in 3- bis 4wöchigen Abständen angeboten (9, 10).
