In the preceding sections, it has been shown how advancement in technology and increased sophistication in the use of and instrumentation for endoscopic procedures has opened the way for the performance of operations which previously had been restricted to an external approach. At times, however, the size, location, or even the nature of the condition precludes an endoscopic correction, and in such instances the surgeon is obliged to proceed via an external route.
