Abstract
After twenty years of experience in stereotactic surgery, we have come to the conclusion that the calculation of the target point from the conventional pneumoencephalography, which is the most innocuous of the methods in use, has the following disadvantages: a) It is not always possible to see the ventricular structures of the midline, third and fourth ventricules and the aqueduct, with sufficient clarity and safety. b) There is considerable asymmetry between the two hemispheres of the patient’s brain. This poses problems when the calculation of the lateral position of the target is based on medially situated points. c) The information on a spatial situation, size and shape of the basal nuclei is obtained from the stereotactic atlases. These are composed of brain sections which do not always correspond to the shape of the whole brain obtained by pneumoencephalography.
