Zusammenfassung
Die Organtransplantation und die Knochenmarktransplantation (KMT) sind heute etablierte Verfahren, um eine gestörte Organfunktion zu ersetzen oder ein hämatologisches Malignom zu heilen [1, 2]. Die Unterdrückung der Organabstoßung ist untrennbar mit einer Störung der Infektionsabwehr verbunden. Aus diesem Grund zahlt der Patient für die neue Organfunktion mit einer erhöhten Infektanfälligkeit. Das Nutzen/Risiko-Verhältnis läßt sich in Zahlen ausdrücken, welche in Tabelle 1 für die Herztransplantation bei terminaler Kardiomyopathie [9] und für die KMT bei akuter myeloischer Leukämie [3] angegeben sind. Die Zahlen zeigen, daß mit der Herztransplantation die beträchtliche Verbesserung der Lebensprognose mit einem hohen Infektionsrisiko bezahlt wird. Bei der akuten myeloischen Leukämie wird das stark reduzierte Rezidivrisiko und das bessere 4-Jahresüberleben mit einer höheren Frühletalität als Folge der 4mal höheren Therapie-assozierten Letalität bezahlt.
