Zusammenfassung
Zu Beginn sollen die Begriffe „Antikonvulsiva“ und „Antiepileptika“ definiert werden; beide Begriffe werden also im folgenden nicht synonym verwendet. „Antikonvulsiva“ verhindern den Ausbruch oder unterbrechen den Ablauf von Konvulsionen, d. h. von anfallsweise auftretenden, spontanen oder ausgelösten unwillkürlichen und unkontrollierbaren tonischen und/oder klonischen Muskelkrämpfen mit Ursprung in abnormen, „paroxysmalen“ Aktivitäten zentraler Neurone. Der Angriffspunkt der Antikonvulsiva liegt im Zentralnervensystem (ZNS), und deren dämpfende Wirkung beschränkt sich nicht auf Krämpfe der Skelettmuskulatur, sondern umfaßt auch die zugrunde liegenden neuronalen Aktivitäten sowie nichtmuskuläre klinische Symptome, wie z. B. anfallsmäßige Veränderungen sensorischer, vegetativer und psychischer Funktionen. Antikonvulsive Wirkungen im beschriebenen Sinne besitzen viele Pharmaka. Der Begriff „Antiepileptika“ ist viel enger. Er bezeichnet Substanzen, die antikonvulsive Wirkungen in Dosen ausüben, bei denen die normalen Funktionen des ZNS und peripherer Systeme nicht oder in erträglichem Ausmaß verändert werden und die sich deshalb klinisch zur Behandlung verschiedener Anfallsleiden eignen. Viele Antiepileptika besitzen neben ihren hemmenden Wirkungen auf epileptiforme Aktivitäten auch psychotrope Eigenschaften.
