Zusammenfassung
Unter seitengetrennter oder differentieller Beatmung versteht man die separate Beatmung der Lungen. Die Trennung erfolgt mit Hilfe eines Doppellumentubus, die Beatmung mit Hilfe zweier Respiratoren. Obwohl die Synchronisierung der Respiratoren physiologisch erscheint und auch wiederholt durchgeführt wurde [3,8,9,14], ist die Beatmung auch ohne Synchronisation ohne nachteilige Auswirkungen [20–22]. Die Beatmung mit nur einem Respirator und speziellen Kreissystemen zur Lungenseparierung wurde zwar versucht [3, 15], konnte sich aber nicht durchsetzen. Neben den unabhängig voneinander wählbaren Tidalvolumina für beide Lungen können mit dieser Methode seitengetrennt verschiedene Niveaus von positiv end-expiratorischem Druck (PEEP) angewendet werden. Die Bedeutung dieser Option ist so groß, daß der Ausdruck „selektiver PEEP“(SPEEP) oft als Synonym für diese Beatmungsform verwendet wird. Die differentielle Beatmung wurde in verschiedensten Situationen eingesetzt, in denen eine einseitige oder einseitig betonte Lungenerkrankung durch konservative Beatmungstechniken nicht adäquat therapiert werden konnte (Tabelle 1). Haben die Lungen bei einer asymmetrischen Erkrankung unterschiedliche mechanische Eigenschaften, so verteilen sich die Tidalvolumina bei konventioneller Beatmung nach der jeweiligen Compliance.
