Biologische Befunde zur Differentialdiagnose von Demenz und Depression

Zusammenfassung

Aufgrund der derzeitigen demographischen Veränderungen mit einem zunehmenden Anteil älterer Menschen an der Gesamtbevölkerung zeigen altersassoziierte Erkrankungen, wie z. B. kardiovaskuläre Erkrankungen und Neoplasien, aber auch psychiatrische Erkrankungen wie Demenzen und Depressionen, dramatisch steigende Prävalenzraten. Im Jahre 1990 gehörten 85.1% der Bevölkerung zu der Gruppe der 0 bis 65-jährigen, 11.1% zu den 65 bis 80-jährigen und 3.8% zu den > 80-jährigen. Im Jahre 2030 wird sich laut bevölkerungsstatistischen Vorausberechnungen eine beachtliche Verlagerung auf Anteile von 73.4%, 20.3% und 6.3% vollziehen (Beske et al. 1994) und somit zu einer weiteren Erhöhung des prozentualen Anteils der älteren Menschen in der Bevölkerung führen und damit auch zu einer weiteren Erhöhung der Häufigkeit der genannten Erkrankungen. Mittlerweile liegt ausreichendes Datenmaterial zu den Häufigkeiten von psychiatrischen Erkrankungen im höheren Lebensalter vor. Eine longitudinale Studie, die in englischen Gemeinden durchgeführt wurde, zeigte altersstandardisierte Prävalenzraten für organische Störungen mit 4.7%, depressive Störungen mit 10.0% und Neurosen mit 2.5% (Saunders et al. 1993). Obwohl die amerikanische „Epidemiologic Catchment Area Study“ einen Abfall der Prävalenzraten für Major Depression im höheren Lebensalter belegte (Regier et al. 1988), gibt es einige Hinweise dafür, daß leichtere Depressionen (Minor Depression), larvierte Depressionen, reaktive Depressionen im Zusammenhang mit somatischen Erkrankungen sowie organische Depressionen höhere Prävalenzraten bei älteren im Vergleich zu jüngeren Bevölkerungsgruppen aufweisen (Angst und Ernst 1993).
