Zusammenfassung
Während antidepressiver Therapie mit klassischen polyzyklischen Antidepressiva (Bunney 1978, Lensgraf 1990, Wehr 1979), sowie unter Lichttherapie (Fleischhacker 1991) und Schlafdeprivation (Kasper 1992) steigt die Wahrscheinlichkeit des Auftretens manischer Symptome bei Patienten mit bipolarer affektiver Störung. Diese Antidepressiva-induzierten Reaktionen können Hinweise auf eine bipolare Erkrankung bei Patienten geben, die zuvor als unipolar affektiv diagnostiziert wurden. In der Literatur werden Fälle von Antidepressivainduzierten manischen Symptomen unter Therapie mit dem selektiven Serotoninwiederaufnahmehemmer (SSRI) Fluoxetin beschrieben (Chouinard 1989, Dunner 1976, Feder 1990, Hadley 1989, Hon 1989, Lebegue 1987, Settle 1984). Wir präsentieren 4 Fälle von SSRI-induzierten manischen Symptomen unter antidepressiver Therapie mit Citalopram bzw. Paroxetin. Nur ein Patient war zuvor als bipolare affektive Störung diagnostiziert.
