Zusammenfassung
Noch vor wenigen Jahrzehnten galten Gewebs-invasive Mykosen als Rarität. Es ist deshalb nicht verwunderlich, daß bis zu diesem Zeitpunkt keine großen Anstrengungen zu Entwicklung von Antimykotika unternommen worden sind. Entsprechend standen dem Kliniker bis zu den fünfziger Jahren nur lokal wirksame Desinfizienzien und schwach antimykotisch wirksame Substanzen zur Verfügung. Im Jahre 1951 entdeckten zwei Angestellte der New York State Division of Laboratories and Research ein Antibiotikum einer Streptomyces-Specieslizenziert mit breitem Wirkspektrum gegen zahlreiche Pilze. Dieses Präparat wurde von E. R. Squibb & Sons als Nystatin (zu Ehren des New York State Laboratoriums) lizenziert. Dieser Erfolg führte zur systematischen weiteren Suche nach Antimykotika durch diese Firma und 1956 zur Entdeckung und Isolation des chemisch verwandten Amphotericin B in Kulturüberständen von Streptomyces nodosus. Trotz hervorra-gender Wirksamkeit blieb der Erfolg des Präparates gegen invasive Mykosen vorerst aus: Amphotericin B wurde kaum resorbiert und auch die Infusion einer Suspension der sehr gut verträglichen wasserunlöslichen Kristalle führte zu ungenügenden Blut- und Gewebsspiegeln. Erst eine galenische Modifikation und Infusion von Amphotericin B als Deoxycholatkomplex zu Beginn der sechziger Jahre ließ das antimykotische Potential dieser Substanz, noch heute die wirk- samste Therapie für die meisten invasiven Mykosen, zur Geltung kommen. Der Wirk- samkeitsgewinn war jedoch mit einer erheblichen Zunahme der Toxizität verbunden. Es ist vielleicht Ironie, daß heute alle Anstrengungen unternommen werden, das Rad der Amphotericin-Geschichte zurück-zudrehen und durch Galenik die Toxizität von Amphotericin B zu vermindern, zum Preis allerdings einer erheblichen Wirkungseinbuße.
