Zusammenfassung
Wie schon in Goethes “Faust” völlig richtig dargestellt, treten schwere psychiatrische Erkrankungen bei Schwangeren seltener auf als bei gleichaltrigen Frauen, die kein Kind erwarten. Trotz mehrerer belastender Lebensereignisse — von Gretchen zumindest mitverschuldeter Tod von Mutter und Bruder und uneheliche Schwangerschaft — wird sie erst nach der Entbindung psychotisch. Tatsächlich steigt die Inzidenz schwerer psychiatrischer Erkrankungen in den ersten Tagen und Wochen nach einer Niederkunft auf ein Vielfaches an im Vergleich zu gleichalten Frauen, die nicht gerade ein Kind zur Welt gebracht haben (Brockington und Kumar 1982). Nach 500 bis 1000 Lebendgeburten in den Vereinigten Staaten und Großbritannien, wurden die Mütter auf eine psychiatrische Station aufgenommen (Brockington und Kumar 1982). Wiederum, wie schon bei Goethe, sind Alleinstehende und Erstgebärende deutlich häufiger betroffen. Nach einer Wochenbettpsychose muß man nach einer von drei bis sieben weiteren Entbindungen mit einer Wiedererkrankung rechnen (Brockington und Kumar 1982). Man sollte die betroffenen Frauen und gegebenenfalls ihre Männer über dieses Risiko aufklären und bei Kinderwunsch eine psychiatrische Begleitung weiterer Schwangerschaften und vor allem Entbindungen anbieten. Dabei erhebt sich gelegentlich die Frage, ob Schwangere Psychopharmaka einnehmen können, ohne ihr Kind zu gefährden. Während des ersten Trimenons muß der Psychiater das Riskio der Teratogenität gegen die Gefahren einer unhehandelten psychiatrischen Erkrankung für Patientin und Embryo abwägen oder gegen die Möglichkeit eines Rückfalls beim Absetzen einer neuroleptischen Erhaltungsmedikation.
