Zusammenfassung
Trotz der hohen Wirksamkeit von L-Dopa als Monotherapeutikum erscheint beim idiopathischen Parkinson-Syndrom unter dem Eindruck der durch das L-Dopa-Langzeitsyndrom bedingten klinischen und therapeutischen Probleme, wie z.B. verstärktes Auftreten von motorischen Fluktuationen, Hyperkinesen und psychotischen Ereignissen nach ca. 5–7 Jahren L-Dopa-Therapie, wird in zunehmendem Maße auch bereits im Frühstadium der Erkrankung zur Verringerung der initialen L-Dopa-Dosis die Kombination von L-Dopa mit anderen Parkinsonmitteln wie z. B. Dopaminagonisten als sinnvoll erachtet. Eine alternative Behandlungsmöglichkeit im Frühstadium der Erkrankung besteht in der Applikation des Monoaminoxidase-B-Hemmers Selegilin (Deprenyl) als Mono- oder Kombinationstherapie In einer multizentrischen Doppelblindstudie (The Parkinson Study Group 1993) konnte die initiale Monotherapie mit Selegilin die Notwendigkeit einer L-DopaTherapie um 337 Tage verzögern.
