Zusammenfassung
Für die Indikation einer Rezidivprophylaxe ist die Frage, wie lange ein primäres Karzinom streng lokalisiert bleibt oder wie früh mit der Ausschwemmung von vitalen Tumorzellen gerechnet werden muß, von entscheidender Bedeutung. Zum Begriff Rezidiv ist festzuhalten, daß dies meist in klinischem Sinn gebraucht wird, wenn ein nachweisbarer Tumor vorliegt. Solange man an der zellulären Genese des Tumorwachstums festhält, ist aber das Rezidiv nur ein gradueller Begriff. Der Patient muß die ganze Zeit seit der Operation, bei der nicht alle Tumorzellen entfernt oder zerstört werden konnten, immer Träger vitaler Tumorzellen gewesen sein. Nur aus solchen Zellen sind durch Vermehrung klinisch faßbare Rezidive oder Metastasen entstanden. Diese Unterscheidung ist wichtig, da es für die Beurteilung der Prognose nicht auf die Nachweisbarkeit von Tumorzellen außerhalb des primären Tumors ankommt, sondern auf den Grad der Wahrscheinlichkeit ihres Vorhandenseins. Wenn nun auch offenbar der Organismus imstande ist, mit einer größeren Zahl von verschleppten Tumorzellen fertigzuwerden, so sind die dabei maßgeblichen Mechanismen noch nicht klar genug, um den Grad ihrer Wirksamkeit in einer bestimmten Situation abschätzen zu können.
