Der Hypothalamus beim Post-partum-Hypopituitarismus

Summary

Patients with severe post-partum hypopituitarism of long duration may develop histological changes of three different types in the hypothalamus.

• 1. About two-thirds of the patients have considerable hypertrophy of the subventricular nucleus. This is proportionate to the degree of atrophy of the uterus. The hypertrophy seems to be related to a disturbance either of the secretory mechanism for gonadotrophin release factors or of the receptor mechanism for oestrogens.

• 2. About half the patients have a late atrophy of the posterior lobe of the pituitary. This is associated with a corresponding atrophy of the supraoptic nucleus, and of some cells of the paraventricular nucleus. In severe cases there is almost complete loss of the mechanism for the neurosecretion of vasopressin, but this does not produce diabetes insipidus.

• 3. When a patient develops hypopituitary coma, small petechial haemorrhages may occur in the hypothalamus, particularly in the vicinity of the third ventricle. After the patient recovers from the coma, these petechiae heal to microscopic glial scars. There are no known clinical effects.

Les malades qui ont subi un sévère hypopituitarisme du post-partum pendant plusieurs années peuvent développer trois espèces de lésions histologiques dans l’hypothalamus.

• 1. Environ deux tiers des malades présentent une hypertrophie marquée du noyau sous-ventriculaire. Celle-ci est proportionnée au grade d’atrophie de l’uterus. L’hypertrophie semble être en relation ou bien avec une perturbation du mécanisme de sécrétion des gonadotrophin release factors ou bien une perturbation du mécanisme réceptoire des oestrogènes.

• 2. Environ la moitié des malades ont une atrophie tardive du lobe postérieur de l’hypophyse. Celle-ci est accompagnée d’une atrophie du noyau supra-optique et de quelques cellules du noyau paraventriculaire. Dans les cas les plus sévères il y a une perte presque totale du mécanisme de sécrétion de vasopressine, mais celle-ci ne conduit pas à un diabète insipide.

• 3. Dans le cas de décès dans un coma hypopituitaire, on trouve parfois quelques petites haemorrhagies dans l’hypothalamus, en particulier aux environs du troisième ventricule. Si la malade guérit du coma, ces petechiae sont remplacés graduellement par des petit noeuds gliaux. Il n’a pas d’effêts cliniques reconnaissables.
