Zusammenfassung

Das diagnostische Bild der abnormen Gewohnheiten und Störungen der Impulskontrolle umfasst pathologisches Spielen, pathologische Brandstiftung (Pyromanie), pathologisches Stehlen (Kleptomanie), Trichotillomanie und andere Gewohnheiten und Störungen der Impulskontrolle. Die Impulsivität als beeinträchtigendes Merkmal bzw. beeinträchtigte psychische Dimension ist ein Symptom zahlreicher Persönlichkeits- und Verhaltensstörungen. In dieser diagnostischen Gruppierung ist sie zentrales Kriterium, wobei hier die verschiedenen abnormen Gewohnheiten wegen gewisser Ähnlichkeiten in der Beschreibung und nicht wegen anderer gemeinsamer Charakteristika zusammengefasst wurden. Definitionsgemäß sind Gebrauch von Alkohol und psychotropen Substanzen, sowie Störungen des Sexualoder Essverhaltens ausgeschlossen. Das bringt uns zu der kontroversiellen Frage der stoffgebundenen und nicht-stoffgebundenen Süchte. Offenbar ist es, in den Augen der Autoren des ICD-10, sinnvoll, den mit Sucht, d.h. mit dem Gebrauch psychotroper Substanzen, verbundenen Verlust der Impulskontrolle diagnostisch und phänomenologisch anders zu fassen als andere, auf den ersten Blick ähnliche, Phänomene. Das schafft allerdings auf der anderen Seite eine, wenig zufrieden stellende, Mischkategorie von allen möglichen Störungen oder Störungssymptomen mit ähnlichem Phenotypus
