Abstract
A 46-year-old patient was diagnosed with a diffuse ductal carcinoma in situ in the right upper outer quadrant. The patient was obese (BMI 35), and her breasts showed a ptosis with the right nipple being 3 cm and the left nipple being 5 cm beneath the inframammary fold. Both of the areolas were larger than 8 cm in diameter and ill demarcated (Fig. 49.1a). The patient desired smaller breasts and autologous reconstruction.
