Zusammenfassung
Ein medizinischer Notfall im alpinen Gelände stellt für die Einsatzkräfte eine spezielle Herausforderung dar. So ist bereits im Vorfeld eine möglichst genaue Einsatzplanung in Zusammenarbeit mit allen an der Rettung beteiligten Organisationen unerlässlich. Die Einschätzung der Gefahrensituation ist dabei von hoher Wichtigkeit. Die eigentlichemedizinische Versorgung richtet sich grundsätzlich nach den international gültigen Guidelines der Notfallversorgung (ABCDE-Algorithmus). Die besonderen Umstände machen jedoch oft ein Abweichen davon notwendig. Die Indikation zu invasiven Maßnahmen ist aufgrund der erschwerten Bedingungen enger zu stellen als bei Notfalleinsätzen im „urbanen“ Umfeld. Blutstillung, Analgesie und v. a. Vermeidung der Hypothermie sollten aber auch unter widrigen Umständen angestrebt werden. Im Falle einer Reanimationssituation wurden von der IKAR MEDCOM zusätzliche Kriterien aufgestellt, die die Entscheidung über Wiederbelebungsmaßnahmen erleichtern.
