Anspruch und Wirklichkeit der Health Professionals in der Arbeit mit älteren Menschen

Zusammenfassung

In der Geriatrie werden häufig Narrationen benutzt, um die Komplexität der Fragestellungen und Lösungsansätze darzustellen. Im Folgenden wird an Hand einer Krankengeschichte das Spannungsfeld zwischen Anspruch und Wirklichkeit von Health Professionals dargestellt. Diese Konflikte sind in der Literatur nur teilweise und oft auch nur narrativ dokumentiert [1], [2].

Eine 82‑jährige Witwe stürzt nach einem abendlichen Besuch bei ihrer Tochter und ihrem Enkel auf der Treppe zu ihrer Wohnung im 1. Stock. Sie kann alleine nicht mehr aufstehen, und nach mehreren Versuchen, Hilfe herbeizurufen, gelingt es ihr unter Schmerzen, ihre Tochter anzurufen, die die Rettung verständigt. Nach ca. einer halben Stunde trifft das Sanitäterteam ein und bringt die Patientin in eine unfallchirurgische Ambulanz. Die Lagerung auf der Transportliege verursacht starke Schmerzen, ebenso der Transport. Nach Aufnahme der Daten kommt sie zur Erstuntersuchung, vom diensthabenden Turnusarzt werden Blut abgenommen, Analgetika verabreicht und die Zuweisung zum Röntgen eingegeben. Die Vigilanz der Patientin verschlechtert sich zunehmend. Mit Hilfe der in der Zwischenzeit eingetroffenen Tochter (diese musste ihrerseits die weitere Betreuung ihres 5‑jährigen Sohnes organisieren) wird mit vielfachen Rückfragen an diese eine Anamnese erstellt.

Die Patientin hat nach ca. 2 Stunden deutlich weniger Schmerzen, ist aber noch immer durch besonders bei Bewegung auftretende heftige Schmerzen beeinträchtigt und zusätzlich durch die analgetische Medikation verlangsamt.
