Zusammenfassung
Entzündliche (z. B. infektiöse Enteritiden, M. Crohn) traumatische (z. B. Abdominaltrauma, iatrogen) und tumoröse Dünndarmpathologien (z. B. Adenokarzinom, NET, GIST) können prinzipiell zu einem akuten Abdomen führen. Nach anfänglich oft unspezifischem Beschwerdebild können die Patienten mit dem klinischen Bild des Ileus, der Hohlorganperforation oder mit einer Blutung auffällig werden. Neben der routinemäßigen Akutabklärung empfiehlt sich meist die Durchführung einer Computertomographie, um mögliche Ursachen des akuten Abdomens eingrenzen und mögliche Differenzialdiagnosen ausschließen zu können. Die Behandlung von akuten Dünndarmerkrankungen umfasst die rein symptomatische Therapie, die krankheitsspezifische konservative Therapie und die Operation, in der – je nach intraoperativem Befund – unterschiedliche Techniken der Dünndarmchirurgie zur Anwendung kommen. Das Outcome wird einerseits durch das akute Abdomen an sich und andererseits durch die verursachende Erkrankung bestimmt.
