Zusammenfassung
Sowohl inkarzerierte Bauchwandhernien als auch spontane Bauchwandhämatome führen zum klinischen Bild eines akuten Abdomens. Patienten mit einer inkarzerierten Bauchwandhernie weisen zumeist eine schmerzhafte Vorwölbung auf, in der Regel wird die Diagnose rein klinisch gestellt. Im Zweifelsfall erfolgt eine CT des Abdomens. Jede Brucheinklemmung ist lebensbedrohlich und verlangt nach einer sofortigen Therapie. Der vorsichtige Versuch einer Reposition kann vorgenommen werden. Ansonsten erfolgt die sofortige chirurgische Intervention. Spontane Hämatome der Bauchwand sind nicht Folge eines Traumas der Bauchwand, sondern entstehen bei Antikoagulanzienbehandlung, Krankheiten mit Blutungsneigung, Schwäche der Gefäßwände und Überstreckung von Muskeln. Das typische klinische Bild ist der akut einsetzende abdominelle Schmerz mit einer tastbaren Verhärtung der Bauchwand. Die konservative Therapie steht im Vordergrund. Nur bei fortbestehender Blutung werden interventionelle oder chirurgische Maßnahmen notwendig.
