Zusammenfassung
Der Typ II Diabetes mellitus ist eine häufige Alterskrankheit. Die Mehrzahl der Typ II Diabetiker in Deutschland hat einen Blutzuckerwert unter 180 mg/dl (entsprechend HbA1c-Werten unter 8%), bei denen weniger als 1% der Patienten einen Endpunkt (Erblindung, Diaiysepflichtigkeit oder Amputation) aufweisen (ZI kassenärztliche Versorgung 2000), Für die Forderung der St Vincent-Deklaration von 1989, einer Reduzierung der Raten an diabetesbedingter Erblindung, Diaiysepflichtigkeit und Amputation bedeutet dies, dass sie derzeit nicht dadurch erfüllt werden kann, dass die durchschnittliche Blutzuckereinstellung bei allen Patienten mit Typ II Diabetes mellitus verbessert wird (Intensivierung der Sekundärprävention), sondern nur dadurch, dass die Minderheit von Risikopatienten mit bestehenden Organschäden rechtzeitig erkannt und dann einer optimierten Betreuung zugeführt werden (Intensivierung der Tertiärprävention (Chantelau 2001)).
