Summary

In 7 single-blind clinical pharmacological trials carried out in healthy volunteers the combination of theophylline ethylene diamine with several sympathomimetic bronchodilators was tested in regard to possible interaction concerning heart rate. The drugs were administered by intravenous infusion (theophylline ethylene diamine, hexoprenaline, orciprenaline, reproterol, salbutamol, terbutaline) or by subcutaneous injection (terbutaline). The trials with hexoprenaline and orciprenaline are reported in detail. The combination of theophylline ethylene diamine with reproterol, salbutamol and especially that with orciprenaline resulted in a stronger increase of heart rate in contrast to the single administration of the respective sympathomimetics. This phenomenon was most obvious following the combination with orciprenaline and a potentiating effect can be assumed. The combination of theophylline ethylene diamine and hexoprenaline in one infusion produced an extreme reduction in the positive chronotropic effect in comparison to the administration of the sympathomimetic alone. The combination of theophylline ethylene diamine and terbutaline showed no difference concerning the increase of heart rate when compared to terbutaline given alone intravenously and terbutaline given alone subcutaneously.

Zusammenfassung

In 7 Untersuchungen an gesunden Probanden wurde die Herzfrequenz-Beeinflussung im Hinblick auf eine mögliche Interaktion von Theophyllin-Äthylendiamin mit verschiedenen Sympathomimetika geprüft. Über 2 Untersuchungen wird ausführlich berichtet. Die Pharmaka wurden intravenös (Theophyllin-Äthylendiamin ; Orciprenalin, Reproterol, Salbutamol, Terbutalin) oder subkutan (Terbutalin) verabreicht. Die Kombinationen von Theophyllin-Äthylendiamin mit Reproterol, Salbutamol und besonders mit Orciprenalin ergaben gegenüber der alleinigen Applikation des jeweiligen Sympathomimetikums eine stärkere Steigerung der Herzfrequenz. Dieses Phänomen war bei Orciprenalin am ausgeprägtesten. Hier kann von einem potenzierenden Effekt gesprochen werden. Nur bei Hexoprenalin ließ die Kombination mit Theophyllin-Äthylendiamin in einem Infusionsbesteck eine Beeinflussung dahingehend erkennen, daß der positiv chronotrope Effekt von Hexoprenalin stark vermindert wurde. Die Kombinationen von Theophyllin-Äthylendiamin und Terbutalin zeigten im Vergleich zur alleinigen Infusion bzw. subkutanen Injektion des Sympathomimetikums keinen Unterschied im Ausmaß der Herzfrequenz-Beeinflussung.
